How can I obtain my medical records?
We are happy to provide you or your physician a copy of your medical records. In order to authorize the release of your medical information, you must complete an Authorization For Use or Disclosure of Protected Health Information release form. For information about obtaining a copy of your psychiatric medical records, please call our office at (918) 453-2160.

How do I obtain the Authorization For Use or Disclosure Of Protected Health Information release form?
You can click here to obtain the release form or you may stop by our office Monday through Friday from 8:00 am to 4:30 pm.

How do I return the release form to your office?
You may choose one of three ways to return the release form to our office. The release form must be completely filled out in order to obtain a valid authorization.

Are there any fees associated with obtaining copies of my medical records?
There is a small charge for copying medical records of $1.00 for the first page and $0.50 per page for the remaining pages. We are happy to send copies of medical records directly to your physician's office at no charge.

Can you send a copy of my medical records to my physician?
We are happy to send copies of medical records directly to your physician's office. You will need to complete an Authorization For Use Or Disclosure Of Protected Health Information release form and provide us with the physician office information. You can obtain the release form by clicking here.

How can I obtain a copy of my baby's birth certificate?
To obtain a copy of your baby's birth certificate, call the Oklahoma Bureau of Vital Statistics in Oklahoma City at (405) 271-4040 and ask for the Birth Records Department.
